Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Only the <br />viewable space will print. <br />Mercy House will provide Rapid Rehousing assistance to 9 homeless persons who will receive security deposit <br />assistance, rental assistance, case managment and housing search and placement services. It Is anticipated that <br />the majority of clients will need medium term assistance (6.12 months). <br />As required by ESG , all referrals for the program will come from the Coordinated Entry System and will have <br />connections to the City of Santa Ana. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12-month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
