Assurances & Certifications <br />Selected providers will be required to sign and submit "actual" assurances and certificates as required by <br />the City of Santa Ann and the Workforce Development Board on all contracts. <br />I recognize that I must give assurances for each item below. Please initial each box indicating you have <br />n read and are providing assurance you are or will be in compliance with thefollowingr <br />l I SAWDB uses the CalJOBs ETPL Module for accepting applications from the providers to be Listed <br />11 �' on the ETPL. Local boards may authorize a single local board to act on their behalf in making <br />determinations for initial and/or subsequent eligibility of providers. Contractors must enter <br />program(s) of training services into CalJOBS. The training provider should only enter the <br />program(s) desired to be on the CA ETPL. If the program is offered with multiple modes of delivery, <br />or course lengths, the program must be entered separately for each variation. <br />V <br />AWDB has authorized the South Bay WDB to make initial and subsequent eligibility <br />eterminations for applications submitted from providers. Contractors should contact the South Bay <br />WDB 11539 Hawthorne Blvd., 51 Floor, Hawthorne, CA 90250, 310-970-7700, <br />CONTRACTOR is required to be in compliance with South Bay WDB Master agreement and <br />SAWDB agreement. <br />n cases where South Bay WDB has denied a provider's application, provider may submit the <br />application to SAWDB for consideration and processing at 801 W. Civic Center Dr. #200, Santa <br />Ana, CA 92701 ATTN: SAWDB Staff. The submission must include completed ETPL training <br />program applications and a copy of the letter from the South Bay WDB denying application. <br />Localboards may establish local policies requiring performance above the state minimum standards <br />"���"'������"""""" for providers to be included on the ETPL. <br />Acceptance and processing of an application does not constitute an agreement or relationship <br />t(IC between the CONTRACTOR and SAWDB, nor does it guarantee any referrals to the provider by <br />SAWDB.
