any proposed Change of Use of the Mobilehome Park until and unless (1) the Planning <br />Commission has approved the Park Owner's application for Change of Use and (2) the Park <br />Owner has signed and filed the certificate accepting the conditions of approval adopted by the <br />Planning Commission. <br />K. Mobilehome Owne Megal Owner Questionnaire A confidential Mobilehome <br />Owner/Resident Questionnaire shall be sent to each Mobilehome Owner/Legal Owner of the <br />Mobilehome Park on a form provided to the Park Owner as soon as the application for Change of <br />Use has been filed. The form shall require the following information for each Mobilehome space <br />in the Mobilehome Park: <br />1. The registered Mobilehome Owner and Legal Owner of the Mobilehome; <br />2. The identity, ages, number, and any mental or physical disability or special needs of <br />the Mobilehome Owner/Legal Owner occupying the Mobilehome; <br />3. The date of manufacture of the Mobilehome, the name of the manufacturer, the size of <br />the Mobilehome, the number of bedrooms in the Mobilehome, any special amenities in <br />the Mobilehome, including but not limited to, equipment needed because of the medical <br />condition, age, or disability of any Mobilehome Owner/Resident in the Mobilehome; <br />4. Any improvements or renovations to the Mobilehome or improvements to the <br />Mobilehome space made by the current Mobilehome Owner/Resident, including but not <br />limited to, a new roof, porches, patios, awnings, pop -out rooms, recreational equipment, <br />barbecue equipment, landscaping, etc., whether such improvements are moveable and the <br />costs of such improvements; <br />5. The purchase price paid by the current Mobilehome Owner/Legal Owner of the <br />Mobilehome and the amount and terms of any remaining mortgage or loan on the home; <br />6. Any special circumstances that would limit the area to which the Mobilehome <br />Owner/Legal Owner is able to relocate; and <br />7. Whether the residents receive any means -tested local, state, or federal government <br />benefit or qualify as extremely low-, very -low, or low- income persons or families under <br />the standards issued by Section 50093 of the California Health and Safety Code, as may <br />be amended, or other comparable federal statute or regulation. <br />The questionnaire shall be kept separately from the rest of the application materials and shall be <br />included in the RIR sent to each Mobilehome Owner/Resident and shall be included in the RIR <br />sent to each Mobilehome Owner/Legal Owner. The identity of each Mobilehome <br />Owner/Resident and his or her responses shall be kept confidential and used only to determine <br />the amount of relocation assistance to be provided to a particular Mobilehome Owner/Legal <br />76 <br />
