H. The SUBRECIPIENT agrees to assist individualsanti families that are homeless or at risk of <br />homelessness In obtaining appropriate supportive services including, but not limited to; <br />temporary and permanent housing, relocation and stabilization services, rapid re -housing <br />assistance, medical and mental health treatment, counseling supeivision, and other servieos <br />essential for achieving independent living. <br />I. SUBR CIPIMNT has agreed to be reimbursed for the above services in an amount not to exceed <br />$25,000 in grant funding for Shelter, Outreach Services, Rapid Re -Housing or homeless <br />Prevention. <br />J, This AGREEMENT is contingent upon the award of ESt3-CV funds from the United States <br />Department of Hm;sing and Urban Development, <br />I{. The CITY and the SUBRECIPIENT have duly executed this AGREEMENT for the expenditure <br />and utilization of said funds. <br />NOW TIIERFXORB, it is agreed by and between the parties that the foregoing Recitals are a. <br />substantive part of this AGREEMENT and the following terms and conditions ace approved and together <br />with all exhibits and attachments hereto, shall constitute the entire AGREEMENT between die Cl'l"Y and <br />the SUBREC1IrIENT: <br />1. SCOPE OF PROM <br />A. General A rinistratiotl <br />The SUBRECIPIFNT agrees to implement this activity as set forth in detail in Exhibit A, Scope <br />of Work, which shalt provide a description of each activity, including the serums to be <br />performed, the person or entity providing the service, the estimated number of recipients of the <br />service, and the manner and means of the services. <br />B, Levels.gfAccompllshment GQaks gllii P©rformance Svle<gtsrarq <br />The SUBRECIPIENT shall be responsible to accomplish the levels of performance as set forth in <br />Exhibit A slid report such measures quarterly to the CITY. If the SUIIRIICIPIENT estimates <br />such goals will not be met, the SU13RECIPIBNT is to contact the CITT, at which time tine CITY <br />will determine if any adjustments to the grant award is appropriatc. <br />C. St ffin. <br />The SUI3RECIPIENT shall ensure adequate and appropriato staffing is allocated to clot, ESG-CV <br />activity. Notivng contained In this AGREEMENT is intended to, or shall be construed in any <br />manner, as creating or establishing the relationship of exuployer/omployee between the parties. <br />II, 'i')i I I OF AGRS,EMFtyT' <br />This AGREEiv1ENT shall be effective froth June 1, 2021, and shall terminate on May 31, 2022, <br />unless otherwise cancelled or modified according to the terms of this AGREEMENT. <br />
