Radioactive materials may be used at temporary job sites of the licensee in areas not under exclusive federal <br />jurisdiction throughout the State of California (see Condition 23). Radioactive materials may be permanently stored <br />only at: <br />(a) 620 S. Magnolia Avenue, Bldg. G, Ontario, CA. <br />(b) 2 Ada, Suite 150, Irvine, CA. <br />(c) 20411 South Susana Road, Suite E, Long Beach, CA. <br />11. This license is subject to an annual fee for sources of radioactive material authorized to be possessed at any one time <br />as specified in Items 6, 7, 8 and 9 of this license. The annual fee for this license is required by and computed in <br />accordance with Title 17, California Code of Regulations, Sections 30230-30232 and is also subject to an annual <br />cost -of -living adjustment pursuant to Section 100425 of the California health and Safety Code, <br />12. Radioactive material may be used only by, or under the supervision and in the physical presence of, individuals who: <br />(a) Have satisfactorily completed a course, accepted by the Department as adequate, in safe use of soil/asphalt <br />content gauges containing radioactive material and also; <br />(b)• Have been provided with a statement, signed by the Radiation Safety Officer, which designates that individual <br />as an authorized user qualified to use radioactive material under this license. Copies of these documents shall be <br />maintained for inspection by the Department, <br />13. Except as specifically provided otherwise by this license, the licensee shall possess and use radioactive material <br />described in Items 6, 7, 8 and 9 of this license in accordance with the statements, representations, and procedures <br />contained in the documents listed below. The Department's regulations shall govern unless the statements, <br />representations, and procedures in the licensee's application and correspondence are more restrictive than the <br />regulations. <br />(a) The renewal application with attachments dated November 13, 2007, signed by Julie Ster, Corporate Radiation <br />Safety Officer, as modified by the letter with attachments, dated July 9, 2009, signed by Julie Ster, Corporate <br />Radiation Safety Officer, the email with attachments received from Julie Ster, Corporate Radiation Safety <br />Officer, on March 15, 2011, and the letter with attachments, dated January 18, 2013, signed by David Robinson, <br />Radiation Safety Officer. <br />(b) The e-mails, both with attachments, received on July 23, 2015 and June 8, 2016, both from Julie Ster, <br />Corporate Radiation Safety Officer, and the letters, both with attachments, dated March 6, 2016 and <br />March 24, 2016, both signed by Julie Ster, Corporate Radiation Safety Officer, in regards to adding <br />20411 South Susana Road, Suite E, Long Beach, CA, as a permanent storage location, releasing 620 West <br />16 Street, Unit 1F, Long Beach, CA, for unrestricted use, Duties and Responsibilities and Delegation of <br />Authority for the Radiation Safety Officer, updated Emergency Contact List, and updated <br />Organizational Chart. <br />14. (a) The Radiation Safety Officer in this program shall be Jeffery Diaz. <br />(b) The Alternate Radiation Safety Officer in this program for Long Beach shall be Roger Khoudessian. <br />(c) The Alternate Radiation Safety Officer in this program for Irvine shall be Farid Ghaheri. <br />15. Sealed sources contained in CPN, Troxler and Humboldt Scientific soil/asphalt gauges shall be tested for leakage <br />and/or contamination at intervals not to exceed twelve (12) months. <br />
