<br />SALE AND INSTALLATION: Subscriber agrees to pay Company the sum of$344.00 , plus $21.62 tax (if applicable), for the sale
<br />and installation of the system listed on me attached "Schedule of Protection." Sum shall be paid $ upon completion of pre -wire, and
<br />s365.62 upon completion of system.
<br />Note: Subscriber acknowledges that (I) additional equipment, at an additional cost, can provide increased detection ability, (n) Subscriber has voluntarily elected
<br />to accept the System based on Subscribers personal reasons, e.g., cost, life style, pets, etc. And (III) your local municipality may require that you obtain a
<br />license or permit forthe installation, use or monitoring of the System and that you are solely responsible for determining and complying with such obligations.
<br />MONITORING: Company agrees to sell or takeover and provide monitoring services (collectively, "Services") for a signaling system ("System"), consisting
<br />of the equipment specified on the attached Rider, which is incorporated herein by reference. Note: Subscriber acknowledges that (I) additional equipment, at
<br />an additional cast, ran provide Increased detection ability, (it) Subscriber has voluntarily elected to swept the System based on Subscribers personal reasons,
<br />e.g., Cost, life style, pets, etc. And (III) your local municipality may require that you obtain a license or permit for the installation, use or monitoring of the System
<br />and that you are solely responsible for determining and complying with such obligations.
<br />Morally Charges:
<br />Subscriber agrees to pay the sum of 125.95 , per month for Monitoring Services, prepaid [jQuarterly [ I Annually DW Three years[ I Five years.
<br />Monthly charge includes'. $15.95 for Monitoring service,$ 10. 00 for Cellular service,$ for Open/Closeservlce, $ farTesttimer
<br />and $ for Quarterly Fire system testing (service). Subscriber also agrees to pay company a one time conversion fee of $ 65.00
