Keep accurate timesheets and submit timesheet to the <br />OCCTAC Youth Employment Program office at the end of each <br />month. <br />D. Immediately report any work related incidents to both the <br />worksite supervisor and the OCCTAC staff <br />E. Communicate any inappropriate behavior of work related <br />issues to OCCTAC staff <br />F. The WEX youth agrees to call the worksite supervisor a <br />minimum of two hours before his/her shift begins if unable to <br />report to work. <br />G. The WEX youth is required to stay in his/her assigned <br />placement for a period of 2-3 months. After a period of three <br />months, if the WEX youth chooses to continue at the worksite <br />he/she may do so with the approval of the work site supervisor <br />and OCCTAC staff. If participation is extended additional duties <br />and opportunities to acquire new skills should be included, in <br />order, to further enrich the WEX youth's experience at <br />worksite. <br />H. Abide by responsibilities outlined in the worksite training <br />agreement. <br />Worksite Supervisor Roles and Responsibilities: <br />A. Be willing to train and mentor the WEX youth <br />B. Be willing to complete quarterly evaluations of WEX youth <br />performance. <br />C. The worksite supervisor understands this to be an entry level <br />position and the responsibilities assigned shall be entry level. <br />D. Be willing to provide to OCCTAC staff and to the WEX youth a <br />Job Description indicating WEX youth job responsibilities at <br />worksite. <br />E. Give WEX youth appropriate safety training for the worksite <br />F.
