INSURANCE NOT REQUIRED return ORIGINAL <br />executed copy <br />WORK MAY PROCEED M-30oCOTC, A-2021-131 <br />CLERK OF COUNCIL <br />CM0(�)(00i55�fa)'F6 MEMORDANDUM OF UNDERSTANDING <br />HEALTH EQUITY AND LITERACY IN OC (HEAL-OC) INITIATIVE <br />This Memorandum of Understanding ("MOU") is hereby made and entered into this 311 day <br />of August, 2021, by and between the City of Santa Ana, a charter city and municipal corporation <br />organized and existing under the Constitution and laws of the State of California ("CITY"), and <br />the Coalition of OC Community Health Centers, Latino Health Access, Orange County Asian & <br />Pacific Islander Community Alliance, and the University of California, Irvine (each individually a <br />"SUBRECIPIENT" and collectively the "SUBRECIPIENTS"). <br />RECITALS: <br />A. In April 2021, the CITY submitted a grant application to the U.S. Department of Health and <br />Human Services ("HHS") for its Minority Health Community Programs to hnprove Minority <br />Health grant funding opportunity. HHS considered the following factors when making <br />recommendations for awarding this grant: applicants serving localities with the highest social <br />vulnerability; applicants with identified Minority Serving Institution partners; and, equitable <br />geographic distribution. <br />B. Working alongside the SUBRECIPIENTS, the CITY successfully submitted a proposal for <br />the Health Equity and Literacy in Orange County ("HEAL-OC") initiative, funded from the <br />Coronavirus Response and Relief Supplemental Appropriations Act, 2021 (P.L. 116-260). <br />The goal of HEAL-OC is to reduce COVD-19 inequities through four objectives: <br />1. Increase collaborative capacity of the CITY and an established partnership, the <br />Orange County Health Equity COVID-19 Community -Academic Partnership. <br />2. Develop a Disparities Impact Statement as a framework for designing, implementing <br />and evaluating interventions. <br />3. Implement a sustainable Health Literacy Plan incorporating National Culturally and <br />Linguistically Appropriate Services (CLAS) Standards. <br />4. Engage with a Minority Serving Institution (UC Irvine) to conduct rigorous evaluation <br />with periodic reporting and quality improvement recommendations to all partners. <br />C. Through these four objectives, HEAL-OC will work to accomplish the following outcomes: <br />1. Capacity — Co -develop with community partners and stakeholders a Disparity hnpact <br />Statement as a framework to guide the development, implementation and evaluation <br />of a Population Health Literacy Plan. <br />2. COVID-19 Education— Develop and implement a Population Health Literacy Plan to <br />deliver linguistically and culturally appropriate COVID-19 education and resources <br />to Latino and AAPI residents in the targeted communities. <br />3. COVID-19 Services — In the first 6 months, achieve a 25% gap reduction in COVID- <br />19 testing and vaccination access for 4' quartile Social Vulnerability hidex areas in <br />Santa Ana, compared to the rest of the county. By the end of the project period, <br />achieve a 50% gap reduction and achieve a 50% increase in the proportion ofresidents <br />with a confirmed vaccination in an information system. <br />4. Health Literacy Surveillance — Establish a Health Literacy Working Group including <br />
