A student requesting to be reinstated as an active student, based on whatever reasons or <br />circumstances, should do so in writing. Supportive documentation and/or information concerning <br />any mitigating circumstances should be noted in the request. The Center appeals board shall <br />consist of (2) two Center administrators The requesting prior student shall be notified of the <br />Reinstatement Review within 30 days following the decision of the Center Director. <br />Graduation Requirements <br />Students will receive the appropriate completion certificate or Diploma if the following <br />requirements <br />have been met: <br />1 All required courses in the student's program have been satisfactorily completed <br />and the final examination has been passed. <br />2 A minimum grade of 80% has been earned in all theory and clinical/ Ride longs <br />coursework, and a Passing grade in clinical/ Ride longs coursework of 80% or <br />better. <br />3 An overall attendance of 80% has been attained <br />4 Financial obligations due Professional Career Development Center have been <br />satisfied in full for the tuition and fees. <br />5 Requirements for administrative departments (financial obligations, and <br />placement <br />information) have been completed satisfactorily. <br />Changing Programs <br />At times, a student may wish to change his chosen career goal. The Center reserves the right to <br />allow or disallowed a transfer from one program to another offered at this Center. For this <br />transfer, the student will sign an addendum to the original contract. The student will be charged <br />for the appropriate time used for the prior program and be charged the additional fees for the new <br />program as the published regulations allow. <br />The process for this program change will be as follows: Notify the Admission Office, Business <br />Office and Student Services prior to the change. Instructors will not make the decision to transfer <br />a student as this effect the student's financial status. <br />Final Exam and License <br />Nurse Assistant and Home Health Aide students, upon completion of the course, and completing <br />the final exam with a grade of 80% on the final examination will be eligible to apply for the <br />certification test given by the State CNA Testing Center. After taking the certification test and <br />passing to receive a certification as a Certified Nurse Assistant with the testing site, the student <br />will be issued a transcript, and diploma, If tuition is paid in full to the Institute. The State CNA <br />certification test is not given by the Center, but by the Regional Testing Center. The school <br />cannot guarantee the State CNA certification. <br />Certified Nurse Assistant Students. upon completion of the course, When the student has <br />completed the Certified Nurse Assistant program she/he can take the American Red Cross Exam. <br />As the student passes the American Red Cross Exam the California Department of Health <br />Services will issue a Certified Nurse Assistant certification. The Certification is mandatory to work <br />in a long term care facility. The Center will then issue the student a Diploma for completion. <br />When the student has received the Certification for the Nurse Assistant, they student can proceed <br />to the Home Health Aide certification with the added theory and clinical for that program. <br />Rationale for the Percentages used for the different major areas <br />Theory Performance Evaluation <br />
