CITY OF SANTA ANA <br />RFP NO.: 21n059 <br />WATER WELL, PUMP AND MOTOR REHABILITATION AND REPAIR SERVICES <br />I Restrooms <br />When applicable, the Contractor shall furnish their own restroom facilities and are <br />responsible for said facilities onsite. <br />4. Storm Water Poliutiott Prevention (SWPP) <br />The Contractor shall not allow any debris from its operations under this Contract to be <br />deposited in the storm drains and/or gutters in violation of the City's NPDES permit. <br />5. Site Maintenance <br />The Contractor shall maintain the site and keep the premises free from accumulation of <br />waste material and the Contractor shall remove all rubbish, implements, surplus material, <br />and temporary facilities generated by the work. <br />6. Cleanup <br />The Contractor is responsible for leaving the work area free of all debris generated by this <br />work at the end of each workday and for final cleanup when the job is completed. <br />The City shall perform inspections of the site prior to the work being performed and identify the conditions <br />of concerns if any remain during and after the work Is completed, <br />19, SPECIAL SAFETY REQUIREMENTS <br />All Contractor's operators shall to observe all applicable State of California Occupational Safety and Health <br />Administration (Cal/OSHA) and Public Works' safety requirements while at Public Works job sites. <br />20. ADDITIONAL RESPONSIBILITIES OF THE CONTRACTOR <br />Contractor and or Contractor's subcontractors must possess and maintain the following active and valid <br />licenses as applicable to individual subcontractor: <br />1. State of California Water Well Drilling Contractor's C-57 License or Contractor's "A" License. <br />2. State of California Limited Specialty Contractor's 0-61, subcategory Machinery and Pump Contractor's <br />D-21 License, <br />3. Have a minimum of five years of experience performing groundwater wells and pump pulling services. <br />If a Subcontractor is used, Subcontractor must also have a minimum of five years of experience <br />performing services as stated above. Also, the Contractor's project manager(s), and Subcontractor's <br />project manager(s), If any, must have a minimum of five years of experience performing groundwater <br />Wells, pump pulling services, video inspections and/or motor repairs. In addition, the Contractor's project <br />manager(s) and Subcontractor's project manager(s), if any, must have the following certifications: <br />• State of California Division of Occupational Safety and Health Administration Hazardous <br />Waste Operations Emergency Response (HAZWOPER) Certification <br />• National Ground Water Association (NGWA) Certification - Pump Installation <br />• Electrical Apparatus Service Association (EASA) Accredited Member Status <br />Provide all labor, equipment, materials, tools, and supervision required to perform as -needed pump pulling <br />
