CITY OF SANTA ANA <br />RFP NO.: 21-059 <br />WATER WELL, PUMP AND MOTOR REHABILITATION AND REPAIR SERVICES <br />APPENDIX <br />ATTACHMENT 2: FEE PROPOSAL <br />Certification - I certify that I have read, understand and agree to the terms and conditions of <br />this Request for Proposal. I have examined the ATTACHMENT 1: SCOPE OF WORK. I am <br />familiar with all the existing conditions and limitation that may impact work requests. I <br />understand and agree that I am responsible for reporting any errors, omissions or discrepancies <br />to the City for clarification prior to the submission of my proposal. <br />Proposer shall submit hourly rates schedule, which shall include but not limited to, direct <br />and indirect costs for labor, for staff per job classification, material, equipment rates, <br />overhead, incidental supplies, travel, mileage, and fuel. Any special materials will be <br />purchased by the contractor only after discussed and authorized by the City projects <br />manager or designee in writing. <br />Prior to commencement of services, Contractor shall provide separate quotes, upon <br />request by the City, which shall be approved by the City's Public Works Water Resources <br />Division. <br />FEE SCHEDULE <br />The undersigned declares that he/she has carefully examined the request for proposal, that <br />he/she has examined the Proposed Scope of Services, and hereby proposes to furnish all <br />material and do all the work required to complete the said work in accordance with said Proposed <br />Scope of Services, for the unit price(s) set forth in the following schedule: <br />Note: This contract is subject to prevailing wages. <br />TO: CITY COUNCIL OF THE CITY OF SANTA ANA <br />FROM: General Pump Company, Inc. <br />MOTOR & PUMP SPECIFICATIONS <br />Motor: US Motor 250 HP, 3 PH, 460 VAC, Frame 445TPA WPI <br />Discharge Head: 12"X12" <br />Existing Pump: Goulds Pump, 16DMC, 3 STG, 298' 7" feet overall length, 12-inch diameter, 10-foot <br />sections <br />Well Casing: 16-inch casing, 1015 feet deep, refer to Appendices for historical information <br />Housing: Concrete housing with hatched roof, refer to appendices for site maps <br />Location: 921 W Walnut St, Santa Ana CA 92703 <br />
