DEC 2 D 1012 <br />CITY OF SANTA ANA <br />PUBLIC WORKS AGENCY <br />CONTRACT CHANGE ORDER <br />Project Number Project No. 16-7566 Raitt-Myrtle Park Change Order Number <br />To _CEM Construction Corporation Contractor <br />You are hereby directed to make the herein changes from the plans and specifications or do the following described work not included in the plans and <br />specifications on this contract. <br />NOTE: THIS CHANGE ORDER IS NOT EFFECTIVE UNTIL APPROVED BY THE CITY COUNCIL OR CITY MANAGER. <br />Unless otherwise stated, rates for rental of equipment cover only such time as equipment is actually used and no allowance will be made for idle time. <br />Change requested by <br />JUSTIFICATION <br />WORKS AGENCY <br />1. To compensate the contractor for costs associated with connecting the new restroom sewer <br />line to a deeper 21" main sewer line per PCO 2.2 dated September 18, 2022. <br />TOTAL COST = $7,442.65 <br />2. To compensate the contractor for costs associated with installing approximately 107 LF of 6" <br />perforated pipe on the south end of the storm chamber, 50 LF of 6" pipe from west area of the <br />storm chamber, to change the dry creek bed area to 1100 SF of bioretention basin, and <br />increase bioretention area by approximately 605 SF per PCO 3.1 dated September 18, 2022. <br />TOTAL COST = $62,141.28 <br />TOTAL AGREED COST: $69,583.93 <br />Page 1 of 2 <br />
