EXHIBIT 2 <br />24 <br />h.SignaturepagethatprovidesthesignatureofboththeHousingAuthorityand <br />theownerandthedates theAHAPwas signed. <br />2.HAP Contract: To be eligible for set-aside funding, the HAP contract must be <br />executed within CY 2021. If the HAP Contract has not been executed by the <br />application submission date, the PHA must state when the HAP contract will be <br />executed. If the HAP contract has been executed by the application <br />submission date, the PHA must provide, fromPART 1 of the HAP Contract <br />forNewConstructionsorRehabilitation,thefollowing: <br />a.The page which identifies the parties to the HAP Contract (both the Housing <br />Authorityand theowner); <br />b.ExhibitAwhichidentifiesthetotalnumberofunitsintheproject coveredby <br />the HAP Contract; the initial Contract Rent to owner, and the number and <br />descriptionofthecontract units; <br />c.If the project is to be completed in stages: in addition to item b. described in <br />phasecovered by theHAP Contract; <br />d.Exhibit B which identifies the services, maintenance and utilities to be <br />providedby theowner; <br />e.ExhibitCwhichidentifiestheutilitiesavailableinthecontractunit,including <br />a listing of utility services to be paid by the owner (without charges in <br />additionto therent toowner)and utilitiesto bepaid by thetenant; <br />f.Section2awhichidentifiestheinitialtermofthecontract;and <br />g.Signature page which provides the signature of both the Housing <br />Authorityandtheowner andthedatestheHAPwassigned. <br />3.CompletedandsignedAppendixI,PBVData(foreachproject-based <br />commitment). <br />2021. <br />E.Category 4 -HUD-VASH: For PHAs administering HUD-VASH vouchers that can <br />demonstrateaneedforadjustmentfundingforatleastoneofthefollowingsituations: <br />1.PUC Increase: For PHAs whose program-wide funded CY 2021 HAP PUC is <br />less than their current HUD-VASH HAP PUC, based on their latest HUD- <br />VASH HAP expenses in CY 2021. HUD will calculate eligibility under this <br />category. Eligibility is determined by comparing the highest HUD-VASH PUC <br />for CY 2021 to the program-wide funded prorated HAP PUC, then multiplying <br />thedifferencebetween thePUCs by thefundedUMLs. <br />2.Leasing Increase: For PHAs whose total HUD-VASH leasing for CY 2021 will <br />exceed the leasing level included in their renewal funding, plus the leasing that <br />will be supported by the RNP and HUD-Held Program Reserves, HUD will <br />calculateeligibilityunderthiscategory.Eligibilityisdeterminedbyadding <br /> <br />
