Lambert, Risk Management Supervisor <br />City of Santa Ana I Human Resources Department I Risk Management Division <br />Ardent Ergonomics appreciates the opportunity of providing the following quote to the City of <br />Santa Ana for completion of preventative ergonomics assessments. <br />Scope of Services <br />Ardent Ergonomics understands the scope of services and can comply with all components. <br />We will: <br />❑ Systematically assess each current Job Description. <br />❑ Determine risk factors and make written recommendations to reduce these risk factors by <br />conducting and providing. <br />❑On -site assessments and modifications <br />❑Employee education <br />❑Ergonomic equipment recommendations <br />❑Equipment Set Up/Training <br />Approach to Project <br />Ardent Ergonomics will: <br />0 Coordinate a time to meet with the City of Santa Ana HR/RM personnel Dept Manager/ <br />Supervisor and employees <br />❑ Observe and each employee performing his or her job, take measurements of workstation <br />including furniture and equipment used to perform job, provide written documentation of each <br />assessment with adjustments and equipment recommendations. <br />❑ Discuss adjustments and equipment recommendations with HR/RM personnel Dept Manager/ <br />Supervisor. <br />❑ Identify and discuss alternate and or modified job duties when possible. <br />Fee rate is $110.00 hr. Professional time, $70.00 hr.
