uJ N.. rt ulUin UUN rcrca ,wU v»N naq Nuw NlN xxm v rcrc iJ d v ml ii i iiiiu u e �N npe <br />N 1 d d i uk n H J N 1 <br />" A A o 0 0 li o <br />G <br />GOAL #2: Reduce gaps in COVID-19 testing and vaccination <br />OBJECTIVE ACTIVITIES/TASKS RESPONSIBLE TIMEFRAME EXPECTED OUTCOMES <br />PERSONS <br />Objective #2.1: in the first 6 <br />months, achieve a 25% gap <br />reduction in COVID-19 testing <br />and vaccination access for the 4th <br />quartile SVI areas, predominantly <br />in Santa Ana and surrounding <br />areas, compared to the rest of <br />Orange County. <br />Objective #2.2: By the end of the <br />project period, achieve a 50% <br />gap reduction, and achieve a <br />50% increase in the proportion of <br />the residents with a confirmed <br />vaccination in an information <br />system. <br />To reduce the existing gap surrounding COVID-19 testing and <br />vaccination access within the 4th quartile SVI areas, we must <br />first address the known barriers, including but not limited to: LHA <br />1. Provide culturally and linguistically appropriate <br />educational material for community members. The <br />lack of knowledge regarding resources availability, <br />hesitancy/questions about the vaccine, or the need for <br />additional community resources has previously been <br />expressed. Primarily, this is because the information <br />received is not linguistically appropriate, or they don't <br />know how to access it. <br />2. Increase the amount of vaccination and testing clinics <br />directly available in neighborhoods (mobile homes, <br />parks, schools, churches, etc.), including evening <br />hours and weekends. <br />3. Reducing the existing digital divide (absence of an <br />email, lack of internet access, help with site <br />navigation). Community members have previously <br />expressed not having a smart device to make <br />appointments with, didn't have an email address or <br />didn't know how to access it, or could not navigate the <br />appointment site due to language or other barriers. <br />4. Educational Material distribution process by <br />Promotores and our collaborative partners (Northgate <br />Gonzalez, SAUDS, AUHSD) via traditional and <br />popular education Outreach as previously described. <br />5. Promote the active COVID-19 vaccination and testing <br />clinics located within or around the geographic areas <br />most impacted by COVID-19 before and during the <br />event. Reaching out to participants during the event, <br />especially at a vaccination clinic, can be effective as <br />the service is available right away for the participant. <br />Moreover, it is essential to create partnerships that <br />facilitate bringing additional vaccination and testing <br />clinics to the 4th quartile SVI areas most impacted by <br />COVID-19. Providing as many vaccination and testing <br />clinics as possible, at various times, within the <br />neighborhoods increases the probability of a resident <br />receiving the service.
