NAME OF PROJECT: Health Equity and Literacy in OC (HEAL-OC) <br />GOAL #1: Increase reach of appropriate COVID-19 resources <br />OBJECTIVE ACTIVITIES/TASKS RESPONSIBLE TIMEFRAME EXPECTED OUTCOMES <br />PERSONS <br />By the end of the LHA and OCAPICA, with the guidance of the Community Community members living within the <br /> <br />project period, SB, will collaborate in the development of culturally and geographic areas most impacted by <br /> <br />4 months in <br />residents in the linguistically appropriate educational material. Focusing COVID-19 will be exposed to culturally <br /> <br />development of <br />targeted on common myths and truths about the COVID-19 and linguistically appropriate educational <br />LHA <br />culturally and <br />communities will vaccine, chronic illnesses considered high-risk, and information regarding COVID-19 and <br />linguistically <br />receive education resource information regarding COVID-19 testing, community resources through strategic <br />appropriate material <br />on COVID-19 and vaccination, and Emotional Wellness amongst other and carefully planned Outreach events. <br />OCAPICA <br /> <br />resources health topics, this educational material will be distributed <br /> <br />By distributing reliable-culturally relevant <br />within the geographic areas most impacted by COVID-19 <br />2 months of training for <br />information, we expect that participants <br />through various forms of Outreach: <br />Community SB Promotores/Community <br /> will have a holistic understanding of what <br />Health Workers <br />is COVID-19 as well as the community <br />1. Promotores (CHW) will canvas these impacted <br />resources that are available and how to <br />neighborhoods by visiting apartment complexes, <br />mobile homes, laundromats, grocery stores, etc. access them. <br />Through traditional and popular education <br />Outreach (costumes, plays, one-on-one <br />conversations, etc.), our Promotores (CHW) work <br />to educate those most vulnerable and isolated <br />within the community regarding various health <br />topics and providing referrals to appropriate <br />community resources. <br />*** Promotores (CHW) will be trained on the educational <br />material to be distributed to best answer any question <br />that may arise while in the field. They will also be trained <br />on various forms of Outreach, how to approach a <br />community member, and how to respond when faced <br />with a non-favorable situation? <br />2. Pop-up booths providing a safe place to ask <br />questions, enquire about community resources <br />and distribute educational material will be set up <br />at predetermined locations throughout the city of <br />Santa Ana (via a special permit acquired) and <br />outside of strategic grocery stores (Northgate <br />Markets). <br />*** A proposal to make flyers, booklets, and other <br />educational material available at intentional locations <br />within the store and providing routine announcements <br />over the store's PA system. <br />3. social media (Facebook & Instagram) will <br />routinely post educational material to promote <br />awareness, education, provide information <br /> <br />
