DocuSign Envelope ID: 098D4B61-B495-429F-BABC-OAF46D368454 <br />Exhibit A <br />Latino Health Access Children and Youth Engagement for Violence Prevention <br />RCP 21-116A REVIVE Santa Ana: Youth Programs <br />Activities <br />Table I below outlines the proposed activities that will support our objectives and outcomes. <br />Activity <br />Description <br />Outreach <br />Latino Health Access will conduct outreach across its partners, including <br />the Santa Ana Unified School District and its parent centers. LHA has an <br />active MOU with SAUSD to provide a variety of services, including a <br />mental health ambassadors program that trains youth to serve as <br />emotional wellness ambassadors. LHA will activate participants of the <br />program, as well as parents and teachers to promote CYE activities and <br />programming. In addition, LHA will leverage its social media platforms <br />as well as local, trusted sources of information such as a Radio Santa Ana <br />Public Awareness <br />Co -designed and co -led with youth, LHA will implement an annual <br />Campaign <br />public awareness campaign that aims to address the various types of <br />violence impacting Santa Ana youth as well as provide resources for <br />prevention and harm reduction. The campaign will leverage youth's <br />creativity and talents to leverage multi -media such as TikTok, short <br />videos, Podcasts, and direct outreach. <br />Monthly <br />Using the concept of "holding space" for youth, LHA and the CYE <br />"Hangouts" <br />youth will organize structured activities and events to elevate <br />issues that are important to youth and are critical for violence <br />prevention and harm -reduction. These can include DJ sets, collective <br />song -writing, healing circles, workshops, and youth -led chats, among <br />other activities that are co -designed by youth. <br />Supportive <br />Leveraging our promotor model, LHA will provide a range of <br />Services <br />supportive services to youth and their families for violence prevention <br />and harm reduction. These may include but are not limited to: emotional <br />wellness services, case management, referrals and linkages, individual <br />and group therapy, connections to food, service navigation, coaching, <br />among others. <br />4. Proposal shall include details of the target population <br />Latino Health Access will prioritize working with Latinx youth in the 92701 Zip Codes through <br />a neighborhood -based approach, although programming will be open to youth across Santa Ana, <br />and it is our goal that as youth increase in leadership to co -implement the proposed <br />programming, they will reach their peers in additional neighborhoods through a peer -engaged <br />model. The youth in our prioritized neighborhoods live in households, which the U.S. Census <br />identifies as having family members who are foreign -born (93%) and who have household <br />incomes of less than $30,000 (88%). <br />21 <br />
