Orange County. The program ensures optimal client health through linkage to medical care, increase client quality <br />of life; and promote client self-sufficiency. Case managers coordinate client use of RHC's many services as well as <br />available community resources that RHC may not provide. <br />Using information obtained during the initial assessment and regular meetings, case managers create an individual <br />service plan customized to the needs and goals of each client. Most importantly, case managers help our clients <br />remain in medical care, which is critical in maintaining their health, reducing their viral load, and the minimizing <br />the number of new HIV infections in the community. Clients remain in RHC's case management program until each <br />of the goals in the clients individual service plan are met, viral suppression is achieved, and self-sufficiency is <br />attained. Case management services are provided daily at our offices, in the home and in other community <br />locations. <br />3). Mental Health Services: Licensed therapists and psychologists provide targeted support groups and individual <br />counseling as well as couples and/or family counseling to assist clients and their families in coping with HIV, mental <br />health symptoms and substance abuse concerns. Mental health services are primarily provided at RHC's office <br />sites. The mental health staff may also travel to client's to reduce barriers to accessing services. <br />4). Nutrition Services: RHC provides groceries, vitamins, and dietary and nutritional supplements for people living <br />with HIV who are low-income or disabled, or seniors to ensure they receive the necessary nutrients to stay healthy <br />and help fight the disease. Clients also receive ongoing education and, when appropriate, referrals to other <br />nutrition services such as Supplemental Nutrition Assistance Program and Women, Infants, and Children. Bus <br />passes are given to those who lack reliable transportation. If a client is too ill to visit our food pantry at our office, <br />volunteers will deliver groceries to the client's home. The Nutrition Services Program operates out of RHC's Irvine <br />site and services are provided daily to clients who are in need. <br />5). Transportation: For clients without reliable transportation, RHC provides Orange County Transit Authority <br />(OCTA) passes, subsidized coupons for use on the OCTA's ACCESS bus system, and subsidized taxi rides and <br />ridesharing. For clients in poor health, RHC's vans and buses transports them to and from primary care, treatment, <br />and social service appointments. Transportation services are provided as needed. <br />6). Housing and Emergency Financial Assistance — We offer financial assistance for rent and mortgage payment, <br />utilities, rental deposits, and transitional housing for clients who are low-income, homeless or at risk of losing their <br />housing due to threat of eviction.
