We will surround the entire family with parenting education and support so the family increases their confidence in parenting skills and positive parenting. Both parents will be provided with classes, education, and support that will teach them coping.skills, resiliency, and strategies for self -care to improve their mental health. They will learn how to recognize the signs and symptoms of anxiety, depression, and other perinatal mood and anxiety disorders, and what to do if recognized. Mothers and fathers will build networks of support and relationships with other moms and dads. Pregnant women and new mothers who are survivors/victims of domestic violence will be provided with supplemental/specialized home visitation, referrals, support, and coaching ng FY24, MOMS' will accomplish the following: • At least 423 Santa Ana mothers, fathers, and babies each year will receive individualized education, support, and/or mental health therapy. • Less than 10% of women will give birth prematurely. • Less than 10% of babies will be born with a low -birth weight. • Less than 10% of babies will be admitted to the NICU. • At least 90% of babies will be on -track to meet important developmental milestones at graduation. • Low-income families will receive health education and promotion of health care and vaccinations. At program exit, at least 90% of babies will have a medical home and at least 90% will be on -track with their vaccinations. • MOMS Orange County will provide approximately 3,300 home visitation encounters for residents of Santa Ana. • Low-income families will receive referrals to community resources that may help them meet their physical and mental health needs, and access basic needs like food, housing, domestic violence support and shelter, and financial assistance. • Low-income families will receive donations of material goods including diapers, wipes, clothing, toys, books, and even car seats as available. • MOMS Orange County will provide 100%of participants with screening for depression/anxiety (once EXHIBIT A
