This model of care <br />addresses the physical, emotional, mental and social aspects of health and well-being of our <br />patients. Our behavioral health services include the following: <br />assessment, and diagnosis, including risk assessment <br />tered treatment planning including risk assessment and crisis planning <br />mental health and substance use services <br />clinic primary care screening and monitoring of key health indicators <br />Targeted case management <br />rehabilitation services <br />support and counselor services and family supports <br />munity-based mental health care for members of the armed forces and veterans <br />: reports encounter data, clinical outcomes data, quality data, and such other data <br />Ath the support from the Santa Ana CDBG, funding will go towards the salary of 1 full-time bi- <br />ngual psychiatric nurse practitioner (PNP) position which will expand services to many more <br />anta Ana residents for psychiatric treatment. The PNP will work a total of 40 hours per week @ <br />197,600 and treat a total of 12 underprivileged individuals a day.
