FEE PROPOSAL <br />P2S INC. <br />CITY OF SANTA ANA <br />RFP NO.: 22-023 <br />ARC FLASH ANAYLSIS AND ELECTRICAL SAFETY PROGRAM DEVELOPMENT <br />APPENDIX <br />ATTACHMENT 2: FEE PROPOSAL <br />Certification - I certify that I have read, understand and agree to the terms and conditions of <br />this Request for Proposal. I have examined the ATTACHMENT 1: SCOPE OF WORK. I am <br />familiar with all the existing conditions and limitation that may Impact work requests. I <br />understand and agree that I am responsible for reporting any errors, omissions or <br />discrepancies to the City for clarification prior to the submission of my proposal. <br />Proposer shall submit hourly rates schedule, which shall include but not limited to, <br />direct and indirect costs for labor, for staff per job classification, material, equipment <br />rates, overhead, Incidental supplies, travel, mileage, and fuel. Any special materials will <br />be purchased by the contractor only after discussed and authorized by the City projects <br />manager or designee in writing. <br />Prior to commencement of services, Contractor shall provide separate quotes, upon <br />request by the City, which shall be approved by the City's Public Works Water <br />Resources Division. <br />FEE SCHEDULE <br />The undersigned declares that he/she has carefully examined the request for proposal, that <br />he/she has examined the Proposed Scope of Services, and hereby proposes to fumish all <br />material and do all the work required to complete the said work in accordance with said <br />Proposed Scope of Services, for the unit price(s) set forth in the following schedule: <br />Note: This contract is subject to prevailing wages. <br />TO: CITY COUNCIL OF THE CITY OF SANTA ANA <br />FROM: P2S, Inc <br />Arc Flash Analysis <br />Fee Schedule for Contractor to provide updated single line electrical drawing, short circuit <br />model drawing and arc flash model drawing for the following locations (existing historical <br />documentation can be found in Appendix B): <br />Item # Bid Item Unit quantity Amount <br />------- - -- <br />1.
