CITY OF SANTA ANA <br />RFP NO.: 22-023 <br />ARC FLASH ANAYLSIS AND ELECTRICAL SAFETY PROGRAM DEVELOPMENT <br />APPENDIX <br />ATTACHMENT 2: FEE PROPOSAL <br />Certification - I certify that I have read, understand and agree to the terms and conditions of <br />this Request for Proposal. I have examined the ATTACHMENT 1: SCOPE OF WORK. I am <br />familiar with all the existing conditions and limitation that may impact work requests. I <br />understand and agree that I am responsible for reporting any errors, omissions or <br />discrepancies to the City for clarification prior to the submission of my proposal. <br />Proposer shall submit hourly rates schedule, which shall include but not limited to, <br />direct and indirect costs for labor, for staff per job classification, material, equipment <br />rates, overhead, incidental supplies, travel, mileage, and fuel.
