HAZMAT TEAM <br />www.hcienv.com <br />www.hcidecon.com <br />Confined Space Entry Event <br />In a confined space entry event HCI Environmental onsite personnel will conduct a pre -entry and rescue site <br />assessment. <br />Once it is determined that a confined space entry needs to be performed, NCI's acting supervisor will fill out a <br />confined space permit. <br />All confined space entrants, attendants and rescue personnel will determine who is going to be assigned what <br />specific tasks during the course of the confined space objective. <br />HCI will perform a lockout/ tag -out procedure on all utilities, if present, in the confined space. <br />All certified confined space equipment such as tripods, harnesses, lanyards, rescue lines, ventilators, SCBA's, <br />winches, and 4 gas meters will be inspected and calibrated. <br />Clearance of the atmosphere will be recorded on the confined space entry permit. <br />A pre -site rescue assessment will be conducted in order to determine the type of rescue that will be utilized. <br />Entry into the confined space will be considered after all 4 gas meter recordings have been performed and are <br />below the Oxygen (o2), Lower Explosive Limit (LEL), Hydrogen Sulfide (H2S) and/or Methane threshold limits. <br />The confined space atmosphere will be consistently monitored during the entry and logged every 10-15 <br />minutes to see if there are any fluctuations in the air monitoring results. <br />After the confined space entry objective has been accomplished the Entry Supervisor will sign off on the <br />confined space permit and close out the confined space permit for the specific entry. <br />
