However, no claim can be paid to any student <br />without a social security number or a taxpayer <br />identification number. <br />Books and Supplies <br />All textbooks and training materials forthe course <br />will be supplied by the school for student use. <br />Students must furnish their own incidental <br />supplies such as pens, paper, notebooks, etc. <br />Note: Classroom reference textbooks are not <br />issued to students but are supplied in sufficient <br />numbers to complete assignments. Books that <br />are lost, mutilated, or not returned timely will be <br />replaced at the student's expense. <br />Students may elect to purchase their own books <br />from outside of the institution and opt -out of <br />receiving books from the institution. <br />Transportation costs <br />Local transportation costs are estimated at <br />$102.00 per month and may vary in specific <br />areas. There are no fees for parking. <br />Delinquent Accounts <br />Delinquent accounts may result in termination <br />from the program and may be submitted for <br />collection with collection agencies. If a student's <br />account is over 120 days delinquent, the account <br />is sent to UEI's internal collection department. If <br />the student's account is still delinquent after eight <br />months, the account is deferred to an outside <br />collection's agency. <br />Arbitration Agreement <br />Arbitration and Class Action Waiver <br />Disclosure: UEI College requires each <br />student to agree to a pre -dispute <br />arbitration agreement and a class action <br />waiver as a condition of enrollment <br />("Arbitration Agreement"). The Arbitration <br />Agreement does not, in any way, limit, <br />relinquish, or waive a student's ability to <br />pursue filing a borrower defense claim, <br />pursuant to 34 C.F.R. § 685.206(e) at <br />any time. The Arbitration Agreement <br />does not require that the student <br />participate in arbitration, or any internal <br />dispute resolution process offered by the <br />school prior to filing a borrower defense <br />to repayment application with the U.S. <br />Department of Education pursuant to 34 <br />21 <br />C.F.R. § 685.206(e). Any arbitration, <br />required by the Arbitration Agreement, <br />tolls (pauses) the limitations period for <br />filing a borrower defense to repayment <br />application pursuant to 34 C.F.R. § <br />685.206(e)(6)(ii) for the length of time <br />that the arbitration proceeding is under <br />way. Any questions about the Arbitration <br />Agreement or a dispute relating to a <br />student's Title IV Federal student loans <br />or to the provision of educational services <br />for which the loans were provided should <br />be directed to the Student Hotline at 866- <br />591-8588. <br />Federal Financial Aid Information <br />Financial Assistance, in the form of grants and <br />loans, is available to eligible applicants who have <br />the ability and desire to benefit from the <br />specialized training offered at LEI College. If a <br />student obtains a loan, the student will have to <br />repay the full amount of the loan plus interest, <br />less the amount of any refund. If the student has <br />received federal student financial aid funds, the <br />student is entitled to a refund of the monies not <br />paid from federal student financial aid program <br />funds, as applicable. <br />To receive financial assistance, you must: <br />1. Demonstrate financial need (with the <br />exception of the Unsubsidized Loan <br />program). <br />2. Be a U.S. Citizen or eligible non -citizen, <br />3. Have a valid Social Security Number. <br />4. If male, be registered with the Selective <br />Service. <br />5. Maintain satisfactory academic progress <br />while attending school. <br />6. Sign a statement that certifies that you will <br />use your federal student aid for education <br />purposes only. <br />7. Not be in default on any federal student loans <br />or owe any money on any federal student <br />grant program. <br />8. Effective July 1, 2012 — Must have a high <br />school diploma, General Education <br />Certificate (GED) or equivalent or qualify <br />under the Eligible Career Pathway Program. <br />Students must complete a Free Application for <br />Federal Student Aid (FAFSA) to assist in <br />determining funding eligibility. <br />
