Express strong motivation, integrity, and desire to <br />pursue their chosen healthcare career through a <br />80 word minimum "Statement of Commitment". <br />4, Meet all institutional and program -specific <br />admissions criteria <br />5. Be enrolled to start at the first available class start <br />date following the scholarship award notification <br />6. Completed scholarship application form <br />CONTINUED ELIGIBILITY REQUIREMENTS <br />1. This is a first come, first serve scholarship so once <br />the Campus limit is attained for the applicable <br />year, this scholarship Is not available, <br />2. Students must graduate to remain eligible for the <br />final disbursement <br />a. If a scholarship recipient does not graduate, <br />or withdraws from school, s/he loses <br />continued eligibility and any award amounts <br />not yet disbursed will be cancelled. <br />3• Continued eligibility is based on the school's stated <br />attendance requirements and the student <br />maintains a 3.0 GPA for the duration of the <br />scholarship award. If attendance requirements and <br />GPA requirements are not met, the remaining <br />balance created due to the loss of scholarship. <br />a. Students who do not meet the Continued <br />Eligibility Requirements outlined above may <br />not re -apply for the scholarship. <br />4• If approved by WSS Committee and all eligibility <br />requirements remain intact, students who have <br />withdrawn in good standing and subsequently re- <br />enter within 18o days, will have their Working <br />Student scholarship funding reinstated, less any <br />prior disbursements. <br />Education Costs <br />Concorde will assist students in developing financial <br />plans to pay for their education through a combination <br />of student or family contributions, financial aid (if <br />eligible), and finance plans. All students must sign a <br />Retail Installment Contract for direct educational costs <br />not covered by Title IV or agency funding. All payments <br />are the full responsibility of the student and are payable <br />as stated in the student's Enrollment Agreement and <br />retail installment contract if applicable. <br />Students may be required to make monthly payments <br />while attending school. Payment amounts are based <br />upon the program in which the student is enrolled and <br />the amount of financial aid the student may be <br />receiving. <br />Any change in financial situation that may affect a <br />student's financial aid or ability to make scheduled <br />payments must be discussed with the Financial Aid <br />Office and the Business Office. <br />Failure to keep all payments current may result in <br />withdrawal from the school, <br />Student Tuition Recovery Fund <br />The State of California established the Student Tuition <br />Recovery Fund (STRF) to relieve or mitigate economic <br />loss suffered by a student in an educational program at <br />a qualifying Institution, who is or was a California <br />resident while enrolled, or was enrolled in a residency <br />program, if the student enrolled in the institution, <br />prepaid tuition, and suffered an economic loss. Unless <br />relieved of the obligation to do so, you must pay the <br />state -imposed assessment for the STRF, or it must be <br />paid on your behalf, if you are a student in an <br />educational program, who is a California resident, or are <br />enrolled in a residency program, and prepay all or part <br />of your tuition. <br />You are not eligible for protection from the STRF and <br />you are not required to pay the STRF assessment, if you <br />are not a California resident, or are not enrolled in a <br />residency program. <br />It Is important that you keep copies of your enrollment <br />agreement, financial aid documents, receipts, or any <br />other information that documents the amount paid to <br />the school. The Institution, a location of the institution, or an <br />educational program offered by the institution was <br />closed or discontinued, and you did not choose to <br />participate in a teach -out plan approved by the <br />Bureau or did not complete a chosen teach -out <br />plan approved by the Bureau, <br />2. You were enrolled at an institution or a location of <br />the institution within the 120 day period before the <br />closure of the institution or location of the <br />Institution, or were enrolled in an educational <br />program within the 120 day period before the <br />program was discontinued. <br />3.
