12.4 Long -Term Disability (L.T.D.) Insurance. The City shall pay one hundred percent (100%) <br />of the premium cost for a sixty (60) day elimination period with a maximum benefit of <br />seven thousand five hundred dollars ($7,500) per month for a long-term disability <br />insurance plan for employees. <br />12.5 Life Insurance. The City shall maintain in effect its existing life insurance plans covering <br />employees, including term life insurance coverage for each affected employee in the <br />amount of fifty thousand dollars ($50,000) plus fifty thousand dollars ($50,000) Accidental <br />Death and Dismemberment (AD&D) coverage at no cost to the employee. <br />12.6 The City shall retain the right to change health, dental, and life insurance carriers, <br />administer the insurance benefits provided thereunder, and select and/or change any excess <br />or supplemental insurance carriers as a part of any self-insurance plan during the term of <br />this MOU, provided that employees continue to receive equivalent benefits and provided <br />that the parties have met and conferred before the benefits changes are made. <br />12.7 When there is a need to discuss matters relating to employee insurance and the City <br />believes it would be beneficial to involve an Insurance Committee, CASA shall have an <br />equal number of representatives as the City on such a committee, and the Committee shall <br />meet as necessary. <br />12.8 Retirement Health Savings Plan. Employees participate in the City's Retirement Health <br />Savings Plan (RHS) subject to the following: <br />A. Employees shall continue to contribute one and one -quarter percent (1.25%) of <br />their base pay each pay period to the RHS plan, which amount shall be deposited <br />into the employee's individual RHS account. <br />Effective the first full pay period following City Council adoption of this 2022- <br />2025 MOU, one and one-half percent (1.50%) will be deducted from each <br />employee's base pay and deposited into their individual RHS account each pay <br />period. This contribution shall be made on a pre-tax basis. <br />B. Effective the first full pay period following Council adoption of this 2022-2025 <br />MOU, the City will contribute one percent (1 %) of the employee's base pay <br />deposited into their individual RHS account each pay period. <br />CONFIDENTIAL ASSOCIATION OF SANTA ANA (CASA) MOU: JULY 1, 2022 THROUGH JUNE 30, 2025 <br />Page 46 <br />
