Licensure <br />Licensure Requirements - Continuing Education Courses <br />Continuing education courses in general do not lead to positions in a profession, occupation, <br />trade, or career field requiring licensure. However, continuing education courses allow <br />professionals to maintain their license on an annual cycle. It is the responsibility of the student <br />to complete appropriate continuing education courses to earn CE's to maintain their licensure <br />and to submit documentation to the relevant agency to support their career field. Some <br />courses under continuing education lead to a Certificate of Continuing Education and are <br />awarded continuing education credit consistent with the approving agency. Other continuing <br />education courses lead to a Certificate of Completion only with no continuing education credit. <br />These certificates allow the license to maintain their license through their appropriate licensing <br />agency. See Appendix B. Completion Documents for Continuing Education Courses. The <br />Director of Clinical Education provides oversight and approval on certificates. <br />Licensure Requirements - Phlebotomy Technician I <br />In order for graduates in the Limited Phlebotomy Technician and Certified Phlebotomy <br />Technician I program to receive a license from the California Department of Public Health — <br />Laboratory Field Services (CDPH-LFS) to be able to work in a phlebotomy/lab setting, all the <br />graduates must apply online with CDPH-LFS. The application requirements include GMC <br />provided documents: program completion certificate, practical training form. The state <br />requirement for licensure also includes completion of a National Certification Exam which the <br />student schedules and receives the certificate. The cost of one sitting for the NCE is Included in <br />the tuition. <br />License Requirements for Limited Phlebotomy Technician applicants does not require a student <br />to take a National Certification Exam. <br />License Requirements for Certified Phlebotomy Technician I applicants does require a student <br />to take a mandatory National Certification Exam. GMC is an approved testing center for <br />American Certification Agency (ACA) and National Healthcare Agency (NHA). <br />In addition, all state applicants must provide an official transcript of high school academic <br />record (transcript) or certified copy of official transcript of GED (General Educational <br />Development), HISET (High School Equivalency Test or TASC (Test Accessing Secondary <br />Completion) result which is sent directly to the state agency. <br />If the student responds "yes" to a conviction on the phlebotomy application, the student must <br />complete a Live Scan Request Form and take it to the Live Scan Center to get fingerprints <br />scanned. The Department of Justice (DOJ) will send CDPH-LFS Phlebotomy an electronic report. <br />The phlebotomy application with the DOJ report will then be reviewed by a committee for <br />approval or additional information. The CDPH-Laboratory Field services (LFS) division reserves <br />the right to deny license to individuals who have been convicted of certain crimes, <br />GMCCatalog-230101 28 <br />
