SERVICE UTILIZATION REPORTS <br />Contractor must provide to the City's Human Resources Department the following reporting in Excel <br />format: <br />1. Quarterly Reports which include at minimum: <br />a. Number of temporary workers hired under this contract; <br />b. Number of hours worked per temporary staff during reporting period, by name; <br />c. Number of hours worked per classification citywide; <br />d. Department to which temporary worker was assigned; <br />e. Billing rate for each temporary worker <br />f. Sum per criteria above <br />2. Annual Report which includes grand totals of each of the criteria included in Quarterly Reports <br />and listed above. <br />
