2022-2025 MOU City of Santa Ana & SEIU Full-Time Employees Unit Page 63 <br />the City will increase the monthly contribution to one hundred thirty dollars ($130) <br />for dental insurance plans. <br />C. Any contribution necessary to maintain benefits under said dental plans in excess <br />of the amounts set forth above shall be borne entirely by the employee. <br />11.3 Long-Term Disability Insurance <br />A. The City shall provide LTD benefit plan coverage with a sixty (60) day elimination <br />period, with a five thousand dollar ($5,000.00) per month maximum benefit, for all <br />employees at no cost to the employee. <br />11.4 Life Insurance <br />A. The City shall provide each employee with a fifty thousand dollar ($50,000) group <br />term life insurance policy and a fifty thousand dollar ($50,000) Accidental Death <br />and Dismemberment (AD&D). <br />B. The City shall provide these life insurance benefits at no cost to the employee. <br />11.5 Benefits Advisory Committee <br />When there is a need to discuss matters relating to employee insurance benefits and the <br />City believes it would be beneficial to involve a Benefits Advisory Committee (BAC), each <br />party (the Union and the City) shall have an equal number of representatives on such a <br />committee to meet as necessary. <br />11.6 Retirement Health Savings (RHS) Plan <br />Employees participate in the City’s RHS Plan subject to the following: <br />1. One and one-half percent (1.50%) will be deducted from each employee’s base pay <br />and deposited into their individual RHS account each pay period. This contribution <br />shall be made on a pre-tax basis. <br />2. Upon an employee’s termination of employment, the cash value of their eligible <br />sick leave balances will be deposited into the individual's RHS account, based on <br />the criteria in Section 10.1(J) and the RHS Plan adoption agreement to provide the <br />employee and their designated eligible dependents, if any, with benefits under any <br />health insurance program including, but not limited to, the program maintained by <br />the City. <br />3. Upon retirement, up to eighty (80 hours) of an employee’s accumulated, but unused <br />sick leave, after eligible RHS payments are made, will be converted into CalPERS <br />Service Credit per Section 12.3(A) and Government Code §20965.
