<br />contribution, if the employee waives, in writing, City-paid coverage. Such waiver <br />shall include a provision warning such employee that reentry into any City- <br />sponsored plan is allowed only at open enrollment, unless there is a qualifying <br />event, and may require proof of insurability for such employee and/or eligible <br />dependents. <br />11.3 Long-Term Disability (L.T.D.) Insurance. For employees covered by this MOU, the City <br />shall pay one hundred percent (100%) of the premium cost for a sixty (60) day elimination <br />period long term disability insurance plan with a maximum benefit of ten thousand dollars <br />($10,000) per month for employee only coverage. <br />11.4 Life Insurance. The City shall continue to pay one hundred percent (100%) of the premium <br />cost for term life insurance coverage under the policy it maintains on behalf of employees <br />in order to provide employees covered by this MOU with life insurance coverage in an <br />amount equal to two (2) times employee's annual rate of salary to a maximum of three <br />hundred thousand dollars ($300,000) provided said employees can provide evidence of <br />insurability of coverage, if so required by the terms and conditions of said term life <br />insurance policy. <br />In the event any such employee is determined to be ineligible for said insurance coverage, <br />the City will attempt to provide as much coverage as may be obtained at reasonable cost <br />not to exceed the actual monthly premium amount allotted for said coverage without having <br />to provide evidence of insurability. <br />11.5 The City shall retain the right to change health, dental and life insurance carriers, <br />administer the insurance benefits provided thereunder, and select and/or change any excess <br />or supplemental insurance carriers as a part of any self-insurance plan during the term of <br />this MOU, provided that employees covered by this MOU continue to receive equivalent <br />benefits and provided that the parties have met and conferred before the benefits changes <br />are made. <br />11.6 Retirement Health Savings (RHS) Plan. Employees participate in the City’s Retirement <br />Health Savings Plan (RHS) subject to the following: <br />A.One and one-quarter percent (1.25%) will be deducted from each employee’s base <br />pay and deposited into their individual RHS account each pay period. This <br />contribution shall be made on a pre-tax basis. <br />B.The City will contribute one percent (1%) of the employee’s base pay deposited <br />into their individual RHS account each pay period. <br />SANTA ANA MANAGEMENT ASSOCIATION (SAMA) MOU: 2022-2025 41
