(9)CITY OF SANTA ANA <br />telephone numbers. Use ATTACHMENT B — References. The respondent grants permission <br />for the City to contact any individuals listed as references. <br />City may disqualify a Proposer if - <br />References fail to substantiate Proposer's description of services and <br />deliverables provided; or <br />• References fail to support that Proposer has a continuing pattern of providing <br />capable, productive, and skilled personnel, or <br />• City is unable to reach the point of contact with reasonable effort. It is the <br />Proposer's responsibility to inform the point of contact(s) of normal City <br />working hours. <br />IX. MINIMUM SCOPE AND LIMIT OF INSURANCE <br />The selected Proposer shall provide the required evidence of insurance coverage as set forth in <br />the standard agreement within ten (10) business days after receipt of Notice of Intent to Award. <br />Contractor must maintain, for the duration of its contract, insurance coverages as required by <br />the City. <br />Additionally, Contractor shall provide the following insurance coverage: <br />Coverage shall be at least as broad as: <br />• Commercial General Liability (CGL): Insurance Services Office Form CG 00 01 covering <br />CGL on an "occurrence" basis, including products and completed operations, property <br />damage, bodily injury and personal & advertising injury with limits no less than $1,000,000 <br />per occurrence. If a general aggregate limit applies, either the general aggregate limit shall <br />apply separately to this project/location (ISO CG 25 03 or 25 04) or the general aggregate <br />limit shall be twice the required occurrence limit. <br />• Automobile Liability: ISO Form Number CA 00 01 covering any auto (Code 1), or if <br />Contractor has no owned autos, hired, (Code 8) and non -owned autos (Code 9), with <br />limit no less than $1,000,000 per accident for bodily injury and property damage. <br />• Workers' Compensation: as required by the State of California, with Statutory Limits, and <br />Employer's Liability Insurance with limit of no less than $1,000,000 per accident for bodily <br />injury or disease. <br />• Sexual Abuse or Molestation (SAM) Liability: If the work will include contact <br />withminors, and the CGL policy referenced above is not endorsed to include <br />affirmative coverage for sexual abuse or molestation, Vendor shall obtain and <br />maintain a policycovering Sexual Abuse and Molestation with a limit no less than <br />$100,000 per occurrence or claim and with a limit no less than $1,000,000 per <br />occurrence or claim for organization. <br />• Professional Liability (Errors and Omissions) Insurance appropriates to <br />the Consultant's profession, with limit no less than $1,000,000 per <br />occurrence or claim, $2,000,000aggregate. <br />If the Contractor maintains broader coverage and/or higher limits than the minimums shown <br />above, the Entity requires and shall be entitled to the broader coverage and/or the higher limits <br />maintained by the contractor. Any available insurance proceeds in excess of the specified <br />minimum limits of insurance and coverage shall be available to the Entity. <br />Page 10 of 56 <br />
