CITY OF SANTA ANA <br />RFP NO.: 22-023 <br />ARC FLASH ANAYLSIS AND ELECTRICAL SAFETY PROGRAM DEVELOPMENT <br />a. Qualified person verification form <br />b. Qualified person in -field and annual inspection form <br />c. Arch flash and shock risk assessment form <br />d. Job briefing form <br />e. Energized electrical work permit <br />f. Contractor/Host employer checklist form <br />The Contractor shall provide any additional recommendations to be utilized in the City's <br />safety program. <br />5. ELECTRICAL SAFETY PROGRAM TRAINING <br />The Contractor shall provide training services to promote electrical safety and the proper use <br />of PPE in accordance with guidelines required in arc flash hazards category 0 through <br />category hazard 4 as well as specified in the customized electrical safety program. The <br />Contractor shall prepare training programs containing but not limited to the following subjects <br />and conduct training annually or upon request: <br />ft �1►� �.Z�]�I�I�liC�7`I <br />i) Terms and definitions (70E and OSHA) <br />ii) Hazards of Electricity <br />1. Shock <br />2. Arc <br />3. Blast <br />2. PERFORMING AN ELECTRICAL HAZARD ANALYSIS <br />i) Determining nominal voltage <br />ii) Shock Approach Distances <br />iii) The Arc Flash Boundary <br />iv) Determining PPE if no Arc Flash Hazard Analysis is performed <br />v) How to choose PPE based on the hazard <br />vi) Hazard vs. risk <br />3. PERSONAL PROTECTIVE EQUIPMENT (PPE) <br />i) Inspection, maintenance and testing of <br />1. Head and hearing protection <br />2. Eye and face protection <br />3.
