Contractor/Host employer checklist form <br />The Contractor shall provide any additional recommendations to be utilized in the City's <br />safety program. <br />5. ELECTRICAL SAFETY PROGRAM TRAINING <br />The Contractor shall provide training services to promote electrical safety and the proper use <br />of PPE in accordance with guidelines required in arc flash hazards category 0 through <br />category hazard 4 as well as specified in the customized electrical safety program. PERFORMING AN ELECTRICAL HAZARD ANALYSIS <br />i) Determining nominal voltage <br />ii) Shock Approach Distances <br />iii) The Arc Flash Boundary <br />iv) Determining PPE if no Arc Flash Hazard Analysis is performed <br />v) How to choose PPE based on the hazard <br />vi) Hazard vs. risk <br />3. PERSONAL PROTECTIVE EQUIPMENT (PPE) <br />i) Inspection, maintenance and testing of <br />1. Head and hearing protection <br />2. Eye and face protection <br />3.
