Approve Local Homelessness Action Plan and HHAP3 Application <br />June 21, 2022 <br />Page 2 <br />The City's final application includes a completed narrative template, a local homelessness <br />action plan, and outcome goals. The approved local homelessness action plan includes <br />the following: a local landscape analysis that assesses the current number of people <br />experiencing homelessness, existing programs and funding which address <br />homelessness, the identification of the number of individuals and families served, <br />demographic subpopulations that are underserved relative to their proportion of <br />individuals experiencing homelessness, and identification of all funds currently being <br />used, and budgeted to be used, to provide housing and homelessness -related services <br />to persons experiencing homelessness or at imminent risk of homelessness. <br />Cal ICH provided the required application templates to be used by jurisdictions seeking <br />HHAP 3 funding. In addition, Cal ICH provided baseline data for the outcome goals <br />spreadsheet from calendar years 2018, 2019 and 2020. The baseline data was prepared <br />to provide standardized baseline data about each Continuum of Care (CoC) on all six of <br />the following required outcome goals: <br />• Reducing the number of persons who become homeless for the first time. <br />• Increasing the number of people exiting homelessness into permanent housing. <br />• Reducing the length of time persons remain homeless. <br />• Reducing the number of persons who return to homelessness after exiting <br />homelessness to permanent housing. <br />• Reducing the number of persons experiencing homelessness. <br />• Increasing successful placements from street outreach. <br />The baseline data was used to assist in identifying underserved populations and in <br />establishing trackable data equity goals as they apply to underserved populations and <br />populations disproportionately impacted by homelessness. <br />In seeking to develop a robust plan informed by local stakeholders, the Homeless <br />Services Division of the Community Development Agency facilitated a community <br />listening session held at the April 27, 2022 Community Development Commission <br />meeting. The meeting was held in person and on Zoom. It was shown on the City's social <br />media platforms and shared with the Continuum of Care mailing list. Feedback received <br />at the community listening session has been consolidated in Exhibit 3. With the guidance <br />of HCFC and the listening session feedback received, staff prepared an application that <br />includes all the required sections and identifies a funding plan to provide individuals <br />experiencing homelessness with pathways for success. These HHAP3 funding plans <br />include the operation and purchase of the Homeless Navigation Center, street outreach <br />services, administration and oversight, and the delivery of permanent housing for youth. <br />Approval of the recommended local homelessness action plan will complete the required <br />functions to submit a HHAP 3 application on behalf of the City of Santa Ana. <br />
