4. CERTIFICATIONS: <br />The following forms shall be signed and included as part of the proposal <br />submittal package: <br />• Attachment 3-1: Non -Collusion Affidavit <br />• Attachment 3-2: Non -Lobbying Certification <br />• Attachment 3-3: Non -Discrimination Certification <br />• Attachment 3-4: Contractor's Licensing and Registration Statement <br />• Attachment 3-5: Prevailing Wage Compliance and Monitoring Statement <br />• Attachment 3-6: Ownership Affidavit <br />• Attachment 3-7: List of Sub -contractors (If included during time of original <br />proposal. Note: As applicable, this list will need to be <br />updated and re -submitted every time a new Task Order <br />is requested by the City from the prime contractor to <br />reflect the scope of work of the requested Task Order.) <br />• Attachment 3-8: References <br />• Attachment 3-9: Statement Regarding Apprenticeship Requirements <br />• Attachment 3-10: Statement Regarding Anti -Kickback Requirements <br />• Attachment 3-11: Public Contract Code Section 10162 Questionnaire <br />• Attachment 3-12: Statement Regarding Community Workforce Agreement <br />(CWA) Requirements <br />V. PROPOSAL REVIEW (CONTRACTOR SELECTION) <br />A. EVALUATION AND RATING <br />The criteria for evaluating the proposals are specified in: <br />RFP SECTION I - INTRODUCTION / PROJECT DESCRIPTION. <br />B. SELECTION <br />The selection committee will be comprised of at least (3) City staff from multiple <br />departments. The committee may or may not interview the top ranking proposers. The City <br />will recommend award of contracts to the proposers who will provide the best quality and <br />value to the City.
