<br /> <br /> <br />City of Santa Ana <br />Athletic Facility Use Allocation Guide <br />September 23, 2021 18 <br /> <br /> <br /> <br /> <br />Please include the following documents with your application: <br /> <br /> Insurance Certificate <br /> Additional Insured Endorsement page or Blanket Endorsement <br /> Player/Participant Waiver <br /> Facility Modification Form; if applicable <br /> Proof of Non-Profit Status <br /> Proof of Residency <br /> <br /> <br /> <br /> <br />A Facility Modification Application (Exhibit 3) is required when a youth sports <br />organization proposes to make any change to a field or playing surface. Submittal must <br />include detailed plans of the work requested, as well as proposed materials used. A n <br />approved Facility Modification Application required by the PRCSA prior to the start of <br />work. <br /> <br />Acceptable modifications include, but are not limited to, adding infield mix, artificial turf, <br />scoreboards, field netting, batting cage, pitching mounds, base anchors, and other <br />apparatus or equipment. <br /> <br /> <br />USER CHECKLIST FOR FACILITY USE APPLICATION <br />FACILITY MODIFICATION APPLICATION INSTRUCTIONS <br />Exhibit 3
