VII. COC ROLE AND RESPONSIBILITIES <br />a. CoC will coordinate and consult with the PHA in developing the services and assistance to be <br />offered under the EHV services fee provided by the PHA and/or their contracted service <br />provider. <br />b. CoC is committed to providing the following services through the CES and the OC Health Care <br />Agency, Office of Care Coordination: <br />i. Educate CES Access Points and CoC Stakeholders on the EHV program, sub- <br />populations priorities for EHV distribution and requirements of this housing <br />opportunity as agreed upon in this MOU. <br />ii. Coordinate the outreach and referral of eligible households through CES. <br />iii. Prioritize households using the CES policies and procedures approved by the CoC. <br />iv. Partner with victim service providers to coordinate referrals through the parallel <br />system for victim service providers. <br />v. Refer eligible individuals and families to PHA through CES, including referrals from the <br />comparable CES for Victim Service Providers. <br />vi. Centralize the application process through the Office of Care Coordination using the <br />universal PHA application and collecting minimally required supporting documents, <br />as detailed and included in ATTACHMENT 2 and 3. <br />c. CoC will provide supporting documentation to the PHA of the referring agency’s verification <br />that the referred individuals and families meet one of the four eligible categories for EHV <br />assistance. <br />d. CoC will strive to match referred individuals and families to the service provider selected by <br />the PHA. <br />e. CoC will comply with the provisions of this MOU. <br /> <br />VIII. PHA ROLES AND RESPONSIBILITIES <br />a. PHA is committed to providing the following services: <br />i. Accept referrals from CES for the EHV program as vouchers are available. <br />ii. Accept direct referrals from outside the CoC CES if: <br />1. the CES does not have a sufficient number of eligible families to refer to <br />the PHA, or <br />2. the CES does not identify individuals or families that may be eligible for <br />EHV assistance because they are fleeing, or attempting to flee, domestic <br />violence, dating violence, sexual assault, stalking or human trafficking. <br />iii. Coordinate and consult with CoC in developing the services and assistance to be <br />offered under the EHV services fee provided by the PHA and/or their contracted <br />Service Provider. <br />iv. Notify applicant, the Office of Care Coordination, CES, and designated supportive <br />service agencies of application and appointment notices, including intake interviews <br />and briefing appointments. <br />v. Provide housing search assistance using available EHV service fee funds and other <br />funding resources, this includes: <br />1. Information and training on what to expect in the EHV issuance process <br />for household. <br />2. Help household identify potentially available units during their housing <br />search. <br />3. Provide transportation assistance and directions to potential units. <br />4. Conduct owner/landlord outreach and engagement. <br />Page 4 of 29 <br />EXHIBIT 2
