Appendix <br />ATTACHMENT 4 <br />CONTRACT COMPARISON FORM <br />RFP NO.21-004 <br />SPILL RESPONSE, HAZARDOUS WASTE DISPOSAL, AND STORM <br />DRAIN FACILITY MAINTENANCE SERVICES <br />Proposers shall complete this form and include it as part of the fee proposal. This form will be used <br />for fee comparison purposes only. <br />LABOR RATES OF CONTRACTOR PERSONNEL: <br />Classification <br />Hourly Rates <br />Straight Time <br />Over -Time <br />Premium Time <br />Project Manager <br />Supervisor <br />Chemist/Industrial Hygienist <br />Lead Technician <br />Equipment Manager <br />Technician <br />All personnel shall have a minimum of 40 hours HAZ-WOPER training as specified by 29 CFR <br />1910-120. <br />NOTE: <br />Straight Time: Monday through Friday, from 7: 00 am to 4: 00 pm. <br />Overtime: Monday through Friday, before 7: 00 am and after 4: 00 pm, and all day <br />Saturday. <br />Premium Time: Sundays, City Holidays and after 12 hours of work. <br />City of Santa Ana RFP 21-004 Page A4-1 <br />
