F. Extension by Use of Accrued Com enp satory Time Off and/or Vacation <br />After an employee's sick leave has been exhausted, they may be granted permission <br />to first use any unused accumulated compensatory time off benefits and then any <br />unused accumulated vacation leave. <br />G. Notification of Use <br />An employee taking sick leave shall notify their immediate supervisor prior to the <br />time they is scheduled to report for duty, or as otherwise established by their <br />department. <br />When the absence is for more than three (3) consecutive working days, upon return <br />to work the employee must present to their Department Head a physician's <br />certificate stating that the employee could not report to work because of such illness <br />or injury, and advising that the employee is sufficiently recovered to safely return <br />to work Such certificate shall be forwarded to the Executive Director of Human <br />Resources. <br />Upon written request by a Department Head or designee, a physician's certificate, <br />or other satisfactory written evidence of the need for qualified sick leave usage, <br />may be required after any future absence of any duration less than three (3) days. <br />Such written notice shall be approved by the Department Head or designee and the <br />stated reason therefore. <br />H. Denial <br />No employee shall be entitled to sick leave with pay while absent from duty because <br />of sickness or injury purposely self-inflicted or caused by willful misconduct; or <br />because of sickness or injury sustained while engaged in employment, other than <br />employment by the City, for monetary gain or other compensation; or due to other <br />reasons resulting from engaging in any business or activity for the purpose of <br />personal monetary gain or other compensation. <br />I. Personal Necessity Leave (Family Sick Leave per Labor Code section 233) <br />Each employee shall be afforded the opportunity to use up to forty-eight (48) hours <br />of sick leave per calendar year, on a non -cumulative basis, as personal necessity <br />leave. <br />1.
