City of Santa Ana REVIVE Scope of Work <br />Program Year 2023-2024 (October 1, 2023 - September 30, 2024) <br />Name of Organization America On Track <br />Name of Funded Program On Track for Gang Prevention <br />Annual Accomplishment Goal <br />Schedule of Performance <br />(estimated) <br />Quarter 1: OCT 1 - DEC 31 <br />Quarter 2: JAN 1 - MAR 31 <br />Quarter 3: APR 1 - JUN 30 <br />Quarter 4: JUL 1 - SEP 30 <br />Unduplicated <br />Participants <br />105 <br />105 <br />105 <br />105 <br />420 <br />Program and Funding Description <br />contract period. <br />4201 Low Income Participant 100% <br />Estimated <br />Invoicing <br />$ 50,000.00 <br />$ <br />50,000.00 <br />$ <br />50,000.00 <br />$ <br />50,000.00 <br />$ <br />200,000.00 <br />AOT will implement a Gang Prevention Services project with youth, ages 10 to 24, throughout the City of Santa Ana. <br />A comprehensive prevention approach, including a mix of evidence -based programs, policies, and practices that <br />address the gang prevention priorities, has been carefully reviewed and developed for our Gang Prevention Services <br />project. <br />Deterring gang involvement and providing pathways for healing and empowerment will be accomplished by <br />implementing the following multi -faceted services: <br />1) Weekly, year-round social justice youth leadership development, life skills, advocacy training and peer mentoring <br />sessions through our Emerging Leaders for Civic Engagement (ELCE) program; <br />2) Weekly, year-round comprehensive services for at -risk youth, including individualized mentoring, as well as healing <br />activities, such as STEAM workshops, yoga, or outdoor activities, and an annual College is For Me Too! summer camp, <br />as well as nutrition education and wraparound services for their guardians, with our Brighter Futures for Children of <br />Prisoners program; <br />3) Multi -week, evidence -based Botvin LifeSkills Training sessions for elementary, middle, and high school students in <br />the highest risk neighborhoods, <br />4) Multi -week, evidence -based CalFresh Health Living direct and indirect Nutrition Education classes for youth and <br />adults in the highest risk neighborhoods, and <br />5) Mental Health Services <br />a. Multi -week trauma counseling for at -risk youth and families who are/have suffered from gang -related trauma. <br />b.
