CITY OF SANTA ANA <br /> <br /> <br /> <br /> <br /> <br /> <br />e. The Vendor shall replace boxes damaged during the transfer from the former’s vendor <br />facility and the new vendor’s facility. Should such damage occur, the vendor shall <br />furnish a report to the SAHA/COSA detailing which boxes were damaged within 2 <br />business days of damage occurring and replace the respective item with like equipment <br />as approved by the SAHA/COSA. <br /> <br />f. Upon successful completion of the transfer, the vendor shall supply to the SAHA/COSA <br />an inventory report of the boxes by department name, reference number and <br />description of the contents as stored. <br /> <br />g. In your proposal, please include the process for acquiring the client’s boxes from the <br />former vendor facility and a time line for the process. Provide policy and procedures for <br />the transition services. Vendor shall provide two cost proposals, one cost for transition <br />of services if no permanent removal (withdrawal) fee is allowed and one if permanent <br />removal (withdrawal) fee is allowed. <br /> <br />MANAGEMENT AND INVOICING <br />a. The Vendor shall provide monthly invoices showing a line item for all the SAHA/COSA <br />storage and activity costs during the month. The invoice shall include a detailed list of <br />all transactions and a summary page listing totals for each department. <br /> <br />b. Vendor may not move SAHA/COSA’s records without prior written notification and <br />approval by SAHA/COSA. Vendor shall notify the SAHA/COSA at least ten (10) day in <br />advance of any changes to vendor’s storage locations. <br /> <br />c. Vendor agrees to pay a penalty of $100.00 per page / record for any lost page / record. <br /> <br />d. Performance and Security Standards: <br /> <br /> The Vendor shall at all times ensure the safety, confidentiality, and integrity of all <br />the SAHA/COSA’s records in its possession. <br /> <br /> The Vendor shall only permit designated SAHA/COSA staff to access and retrieve <br />the records stored at its facility. The SAHA/COSA will provide the Vendor a list of <br />staff authorized to access the records and notify the Vendor in writing of any <br />changes to the list. <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br />EXHIBIT 1
