<br />Q. How many of those 2023 Street Exits were permanent exits? <br />A. City Net handles the warm hand off of an unhoused individual to the shelters, rehabilitation <br />locations, etc.., and then keeps records on that street exit. To be able to track permanent <br />exits, shelter providers, rehabilitation facilities, etc.., would need to have a willing shelter <br />client/unhoused patient that wants to keep in touch with the shelter or drug rehabilitation <br />facility. A case management process that creates a follow-up phase would need to be <br />implemented by a shelter operator or rehabilitation facility operator. <br />Q. How many of those 2023 Street Exits were attributed to individuals that entered, then exited, then <br />re-entered shelter/housing multiple times? <br />A. It is not in City Net’s purview to track unhoused individuals long-term. A follow-up phase as <br />part of case management would need to be implemented, but this would be difficult because <br />we have found that phone numbers, addresses, etc.., change for unhoused individuals over <br />time because they lack permanent stability. <br /> <br />5 <br />
