(9) <br />CITY OF SANTA ANA <br />• Animal's name, age, breed, and weight; and <br />• Include information regarding any complications, issues, or other pertinent information <br />regarding provision of service; <br />• All additional information required to show proof of service and costs. <br />9) Submit prior month's report no later than the tenth (101h) day of the following Month. Monthly <br />reports shall be delivered to the following address: <br />City of Santa Ana <br />Code Enforcement <br />20 Civic Center Plaza-M19 <br />Santa Ana, CA 92702 <br />10) All documents, records, application, files, and other materials provided in connection with <br />services rendered under this agreement shall be the property of the City of Santa Ana <br />and shall be provided to the City within seven (7) business days, upon request and/or <br />upon the termination of this agreement. <br />11) The City shall have the right to audit the books, records, and accounts that are related to this <br />agreement at any time and at its own discretion. <br />City of Santa Ana RFP 23-180 Page 20 of 35 <br />
