(Cancellation) of the policy is replaced by the following: <br />D.Cancellation and Nonrenewal <br />1.Youmaycancelthispolicy.Youmustmailordeliveradvancewrittennoticetousstatingwhenthecancellationis <br />to take effect. <br />2.If you cancel or fail to renew this policy, we must promptly notify the Industrial Commission of Arizona. <br />3.Wemaycancelthispolicyifyoufailtopaypremiumwhendue,orwhenoneorbothofthepartiestoa <br />professional employer agreement terminate the agreement. <br />oIfwecancelornonrenewthispolicy,wemustprovidetoyouandtheIndustrialCommissionofArizonaat <br />least 30 days' notice of the cancellation or nonrenewal. <br />oNotice to you may be sent via mail or delivered by electronic means as follows: <br />oMailingthatnoticetoyouatyourlast-knownmailingaddressonfilewithuswillbesufficientproofof <br />notice. <br />oDelivery to an email address at which you have consented to receive notices or documents. <br />oPostingonaportal,securewebsite,electronicnetworkorsiteaccessibleviatheInternetoramobile <br />application,computer,mobiledevice,tablet,orotherelectronicdevice,togetherwithaseparatenotice <br />thatincludesadescriptionofthedocumentornoticethatwaspostedandthatwasprovidedbyemailto <br />theemailaddressatwhichyouconsentedtoreceivenotice,orbyanyotherdeliverymethodtowhichyou <br />consented. <br />oIfyouconsentedtohavethenoticeemailedinaccordancewithArizonalaw,emailingthatnoticetoyouat <br />your last-known email address as provided by you to us will be sufficient proof of notice. <br />oIftheemailnoticeis:(1)rejectedfordelivery;(2)returnedtous;or(3)webecomeawarethatthe <br />emailaddressprovidedbyyouisnolongervalid,thenwewillalsomailthatnoticetoyoubyUS <br />PostalServicecertifiedmail,certificateofmailing,orfirst-classmailusingintelligentmailbarcode,or <br />another similar tracking method used or approved by the US Postal Service. <br />oIfwenonrenewthispolicyandfailtogiveyounoticeofnonrenewal,coveragewillnotextendbeyond <br />the policy period. <br />4.The policy period will end on the date and time stated in the cancellation or nonrenewal notice. <br />5.Anyoftheseprovisionsthatconflictwithalawthatcontrolsthecancellationoftheinsuranceinthispolicyis <br />changed by this statement to comply with the law. <br />Form WC 02 06 01 CPrinted in U.S.A. <br />Process Date:08/17/24Policy Expiration Date:09/26/25 <br /> <br />
