served through our program, 92% were developmentally on-track at closure, 93% had completed <br />doctor-recommended vaccination schedules, and 100% had a medical home with a pediatrician upon <br />graduating. <br /> <br />As of early 2024, the Maternal Child Health Coordination program has rendered its services in <br />partnership with CalOptima through its Community Health Worker and Enhanced Care Management <br />initiatives under CalAIM.
