Exhibit A SCOPE OF SERVICES A. Provider will conduct Danza Folkl6rico class for individuals 4yrs & Up B. Provider shall teach such or similar classes (1) at times below at facilities to designated by the city or (2) on a schedule agreed upon by the parties for each class session or term, including the location, specific days and hour when classes will be held, and holidays to be observed, in accordance with City's needs. INSTRUCTOR: Dennys Diaz Padilla, Substitute (Maricsa Secundo) LOCATION: El Salvador Center, 1825 W. Civic Center Dr., Santa Ana, (714) 647-6558 • Danza Folklorico (Beginning) class that will consist of monthly sessions, held I day per week, 1 hour per day. (4yrs+), 5:30pm-6:30pm Mon/Wed, $55 monthly; 12:00pm-I:OOpm Sat, $45 monthly. • Danza Folklorico (Intermediate) class that will consist of monthly sessions, held 1 day per week, 1 hour per day. (6-12yrs), 5:30pm-6:30pm Mon/Wed, $55 monthly. • Danza Folklorico (Adaptive Dance) class that will consist of monthly sessions, held 1 day per week, 1 hour per day. (5yrs+), 5:30pm-6:30pm Mon/Wed, $45 monthly. * For students of all ages and abilities. • Danza Folldorico (Advanced) class that will consist of monthly sessions, held I day per week, 1 hour per day. (12yrs+), 5:30pm-7:30pm Mon/Wed, $55 monthly. C. Provider will provide and be responsible for equipment, records, and personnel and cleanup of the facilities and materials necessary to ensure the safety and effectiveness of said instruction. CLASS SIZE/Registration 1) Each class must have a minimum of 2 paid students and no more than a maximum of 20. 2) No registration accepted after the second meeting of class. 3) In the event the minimum number of enrollees has not realized by the second meeting of the class, the class will be canceled. Consultant will be under no obligation to provide services and the City will have no obligations to pay Consultant compensation CLASS FEES • Each participant shall pay class registration fee as established by City • Provider may not waive class participation/registration fees. • Only registered participants may participate in class. • Any refunds to participants will be made in accordance with City policy. • Any material fee shall be established by mutual agreement of City and Provider shall be payable directly to Provider.
