The participant must provide itemized receipt(s) totaling the amount provided within 30 days that <br />the gas card is provided and/or before receiving additional gas cards, and the itemized receipt(s) <br />must then be included in the participant's file. If not returned, no additional supportive services may <br />be provided. <br />2. The case manager must perform and record in Ca1JOBS case note if a participant fails to return <br />the receipts for the gas cards; the case manager must contact the participant requesting the missing <br />receipts and explain that no further supportive services will be provided until the missing receipts <br />are received in full; and <br />3. If the receipts are not provided, send a follow-up letter within five (5) business days to the <br />participant requesting the receipts and notifying the participant that no further supportive services <br />will be provided until all receipts are received. A copy of this letter must be kept in the participant's <br />file. <br />Fingerprinting —Live Scan and TB Test: <br />Participants who work with children and are placed in an educational setting will be required to <br />undergo Fingerprinting —Live Scan through the Department of Justice and show a cleared TB Test. <br />This supportive service will be available for participants on a case -by -case basis and determined by <br />the case manager. <br />Work and School Related Materials: <br />A participant may need to purchase books, fees, school supplies and/or other necessary materials to <br />enable him/her to participate in activities leading to a credential. A participant may need to purchase <br />work related equipment, attire, tools, or certification for the purpose of obtaining employment and <br />to enable him/her to be successful in employment. <br />Child/Dependent Care: <br />After identifying the need for assistance and eliminating other resources, the case manager and the <br />participant will develop a service plan (ISS) and update progress in the case notes. <br />Childcare assistance is defined as the provision of $75 payment to institution/daycare/childcare <br />provider upon receiving an invoice. <br />2 <br />Attachment C Revised: 07/10/2024 <br />
