ARTICLE XI <br />11.0 EMPLOYEE INSURANCE <br />11.1 Health Insurance. The City shall contribute toward the <br />payment of premiums for affected employees and their <br />dependents under the Ca1PERS Health Program. Effective <br />January 1, 2002, January 1, 2003, and January 1, 2004, <br />respectively, the City shall contribute toward medical <br />premiums an amount consistent with the rates in effect for <br />each tier of the Kaiser California Ca1PERS HMO plan. For <br />example, effective Janaary 1, 2002, the California Kaiser <br />rate for the "employee only" category is $210.17 per month, <br />the rate for employee plus one is $420.34 per month, and the <br />rate for family coverage is $546.44 per month. <br />For each such employee who is covered under a spouse's non - <br />City sponsored health plan, the City will pay the employee <br />a cash payment each month in an amount equal to fifty <br />percent (50%) of the monthly premium amount for the City's <br />lowest "employee -only" coverage, if said employee waives, in <br />writing, City -paid coverage. <br />11.2 Dental Insurance. During the term of this Agreement, the <br />City will contribute the following amounts for each affected <br />employee enrolled in dental insurance plans provided by the <br />City for its bargaining unit members and their eligible <br />dependents: <br />Effective <br />1-1-02 - <br />1-1-03 <br />up <br />to <br />$60/month <br />per <br />employee <br />Effective <br />1-1--03 - <br />1-1-04 <br />up <br />to <br />$70/month <br />per <br />employee <br />Effective <br />1-1-04 - <br />1--1-04 <br />up <br />to <br />$80/month <br />per <br />employee <br />Any contribution necessary to maintain benefits under said <br />dental plans in excess of the amount set forth above shall <br />be borne entirely by the employee. <br />11.3 Disability Insurance. The City shall continue to pay one <br />hundred percent (10051.) of the premium cost for a long term <br />disability insurance plan for employees covered by this <br />Agreement. <br />11.4 Life Insurance. The City shall continue to pay one hundred <br />percent (1000-0 of the premium cost for term life insurance <br />coverage under the policy it maintains on behalf of its <br />officers and employees in order to provide employees covered <br />by this Agreement with life insurance coverage in an amount <br />equal to twice such employee's annual rate of salary to a <br />maximum of three hundred thousand ($300,000) provided said <br />affected employees can provide evidence of insurability of <br />Coverage if so required by the term-, and conditions of said <br />term life insurance policy. <br />19 <br />
