c) <br />INSURANCE ON F11 1�� <br />WORK MAY PROGEEC) <br />UNTH, MSU7ANCE EXPIRES <br />1"Oo <br />EE —5; 11 K 61-- 5HREE . . ..... —,—AMENDMENT TO CONSULTANT AGREEMENT <br />DATE - <br />THIS AMENDMENT TO AGREEMENT made and entered into this 1--� day of <br />"'ITI-1 L*_ 2002, by and between the City of Santa Ana, a charter city and <br />municipal corporation of the State of California ("City") and Nicole Maiocco ("Consultant"). <br />RECII'ALS: <br />A. City and Consultant entered into a Consultant Agreement, dated July 15, 2001 by <br />which Consultant agreed to provide domestic violence counseling services (said <br />"Agreement") <br />B. Since the date of said Agreement, Consultant has changed her name from Nicole <br />Porter to Nicole Maiocco. <br />C. The parties desire to extend the term of said Agreement to December 31, 2002. <br />WHEREFORE, in consideration of their mutual and respective promises, and subject to <br />the terms and conditions of the Agreement as hereby amended, the parties hereby agree as <br />follows: <br />1. Section 3 of the Agreement is hereby amended to read as follows: <br />"This Agreement shall commence on July 15, 2001 and terminate on December 31, 2002, <br />unless terminated earlier in accordance with section 12, below. The terms of this <br />Agreement may be extended upon a writing executed by the Chief of Police and the City <br />Attorney." <br />Page 1 of 2 <br />
