Consolidated Annual Performance and Evaluation Report for FY 2023-24 <br />September 23, 2024 <br />Page 2 <br />4 <br />5 <br />3 <br />1 <br />Consolidated Annual Performance and Evaluation Report (“CAPER”) (Exhibit 1). The <br />CAPER describes the activities and accomplishments for the three federal entitlement <br />grants during the past Fiscal Year. It also reports on the City’s success in achieving the <br />five-year numeric goals and objectives that are established in the Five-Year <br />Consolidated Plan. Fiscal Year 2023-24 is the fourth year covered by the July 1, 2020 <br />through June 30, 2024 Consolidated Five-Year Plan. <br />The following table highlights accomplishments achieved during Fiscal Year 2023-24 <br />with funding from the CDBG, ESG, and HOME Program: <br />General Activity Accomplishments <br />Capital <br />Improvements <br />•175,690 residents were assisted through the improvements of <br />neighborhood facilities, parks and recreation facilities, and street <br />improvements. <br />Economic <br />Development •Provided 12 small business incentive grants to small businesses. <br />Public Services <br />•Provided funding for 20 nonprofit organizations to administer 21 <br />programs and served a total of 4,255 persons. <br />•America on Track, Boys and Girls Club, Delhi Center, Girls Inc., <br />Neutral Ground, Orange County Children’s Therapeutic Arts <br />Center, Taller San Jose, and The Cambodian Family specifically, <br />provided youth services that included tutoring, education, <br />recreation, fitness, a classical music program, and gang <br />prevention. <br />•Community Action Partnership of Orange County and the Delhi <br />Center provided youth and family services that included financial <br />empowerment, education, leadership, and financial workshops. <br />•Casa de la Familia, Community Legal Aid SoCal, Lutheran Social <br />Services of Southern California, and Human Options provided <br />criminal record expungement and domestic violence services that <br />included housing and legal services. <br />•Community Health Initiative of Orange County, MOMS Orange <br />County, Neutral Ground, and Radiant Health Services provided <br />access to health and social service programs by conducting <br />outreach, education, and enrollment. <br />•Stand up for Kids, WISEPlace, and Public Law Center provided <br />affordable housing and homelessness services. <br />Affordable Housing <br />•The Westview House affordable housing project located at 2530 & <br />2534 Westminster Avenue, Santa Ana, CA 92701 continued <br />construction. This project will provide 84 units of affordable <br />housing for large families including nine (9) HOME assisted units. <br />•The Crossroads at Washington affordable housing project located <br />at 1126, 1136, and 1146 E. Washington Avenue continued <br />construction. This project will provide 85 units of affordable <br />housing for large families including 12 HOME assisted units. <br />•The WISEPlace Permanent Supportive Housing project located at <br />1411 N.
