Name and contact information of the supervising Project Manager/Principal <br />Agent, to be assigned to the agreement. The Project Manager/Principal <br />Agent shall be the primary contact person to represent your firm and will be <br />the person to conduct the presentation, if invited to an interview. <br />iv. Resumes for all key staff proposed describing relevant experience. <br />e. Proposed Work Plan/Understanding of Need <br />Proposal shall include a statement demonstrating the firm's understanding of the <br />Scope of Services. <br />Additionally, proposed work plan shall include Proposers': <br />i. Anticipated approach to performing services as specified herein; <br />ii. Suggestions or special concerns the evaluation committee should take into <br />consideration (if any); <br />iii. Description of deliverables and implementation plan. Proposer shall submit a <br />general description of the deliverables, implementation plan, and timeline. <br />References: Proposals shall include a listing of relevant projects with references for <br />three public entities with valid current emails for which Proposer has performed <br />similar work within the past five (5) years. <br />g. CARB Fleet Compliance <br />The City is a Public Works Awarding Body, as defined under Title 13 California Code <br />of Regulations section 2449(c)(46). Accordingly, Proposers must submit, with their <br />Proposals, valid Certificates of Reported Compliance ("CRC") for the Proposer's fleet <br />and for the fleet(s) of its listed subcontractors (including any applicable leased <br />equipment or vehicles). Proposer must additionally complete and submit the Fleet <br />Compliance Certification, included herein as ATTACHMENT H. Failure to provide a <br />CRC for the Proposer, and for all listed subcontractors, or failure to complete the Fleet <br />Compliance Certification, may render the Proposal non -responsive. <br />2. Cost Proposal <br />All Proposers are required to submit a fixed rate fee with their Cost Proposal. Pricing <br />instructions should be clearly defined to ensure fees proposed can be compared and <br />evaluated. Cost Proposal must include a payment schedule if applicable. City reserves <br />the right to negotiate compensation and/or payment schedule prior to award of any <br />resulting agreement. When applicable, if providing hourly rate sheets, Proposer shall not <br />include rate ranges or averages. <br />The fee proposal shall be submitted separately and concurrently with the technical <br />proposal, electronically in PlanetBids, and as a hard copy in a separately sealed <br />enveloped, clearly labeled as "Fee Proposal." This shall include the firm's Standard <br />Hourly Fee Schedule, a table outlining the tasks and team hourly effort for each of the <br />major tasks, and a Project Fee Schedule, as outlined in the Scope of Work. <br />
